9. Last 5 years/52 weeks tax returns
Start keeping a diary of how your medical condition affects your daily life. The more detail the better. Start with how you sleep and work through an entire day. Do this for a month. This will greatly improve your ability to give testimony and increase your chance of being granted benefits under the legislation.
